Maintenance treatment and shared care in lymphoblastic leukaemia.

چکیده
Treatment of children with acute lymphoblastic leukaemia (ALL) and non-Hodgkin's lymphoma has become more intensive and more successful in recent years. The first few months from diagnosis are punctuated by repeated admissions to hospital for periods of intensive chemotherapy, and often by readmission for management of the resulting fever and neutropenia. However, over half of the treatment period for virtually all children with ALL and for many with non-Hodgkin's lymphoma of T cell subtype is spent receiving oral drugs as an outpatient and during this phase of 'maintenance' treatment shared care between the tertiary centre and the local paediatric unit comes into its own. How often should children have blood counts during maintenance treatment? What is the risk of infections and how should they be managed? What is the risk of chickenpox or measles? What advice should be given at the time of stopping treatment? These are some of the many questions asked by parents, paediatricians, and family doctors. In this article the author reviews her approach to these problems which arise during maintenance or continuing treatment.

Gene variants of CYP1A1 and CYP2D6 and the risk of childhood acute lymphoblastic leukaemia; outcome of a case control study from Kashmir, India
Studies on associations of various polymorphisms in xenobiotic metabolizing genes with different cancers including acute lymphoblastic leukaemia (ALL) are mixed and inconclusive. The current study analyzed the relationship between polymorphisms of phase I xenobiotic metabolizing enzymes, cytochromes P450 1A1 (CYP1A1) and CYP2D6 and childhood ALL in Kashmir, India. We recruited 200 confirmed ALL...
متن کاملLong-term results of national treatment trials in childhood acute lymphoblastic leukaemia in several Middle/East European countries
Acute lymphoblastic leukaemia (ALL) is the most frequent childhood cancer. During the last 50 years its prognosis has improved from zero probability of survival in the 1950s to more than 80% chance to be cured at the beginning of the 21st century. ALL is a highly chemosensitive disease and its successful treatment is a good indicator of the level of medical care that children with cancer get in...
متن کاملSignificance of Cross Lineage Antigen Expression in Acute Lymphoblastic Leukemia
Background: Aberrant expression of cross-lineage antigens gives valuable insight into the diagnosis and prognosis of acute leukemia. In countries like India, cytogenetic tests are widely accessible. Exploring the prognostic value of an accessible test is of great importance. Therefore, establishing a population-specific immunophenotype database will enable to design an antibody panel equipped t...
متن کاملLISA: A Clinical Information and Decision Support System for Collaborative Care in Childhood Acute Lymphoblastic Leukaemia
The treatment of a child with Acute Lymphoblastic Leukaemia (ALL) requires the collaboration of multiple providers across different organisations. We describe the implementation of a clinical information system for supporting collaborative care in the management of children with ALL.
